Transitioning patients to a home-based environment is the focus for transformational healthcare!  Two healthcare experts, Dr. Josh Luke and Dr. Karen Vanaskie, will provide this 4.0 CEU presentation with focus upon new expectations transitioning from the acute hospital setting to home, technology advancements, and a shift from In-Patient to Out-Patient environments!

By attending this learning session, participants will:

  1. Attendees will understand the shift in control from the hospital sector to the insurance sector and how home based care will be the focus of the industry.
  2. Attendees will understand that hospitals are shifting from profit centers to the largest expense in a bigger model, and discharging patients home will be the first priority.
  3. Attendees will learn about the Discharge with Dignity Model, as well as new technologies allowing quick, safe transfers that enhance data capabilities. 
  4. Identify three best practices in transitioning care from Inpatient to Outpatient.
  5. Explain several ways effective communication improves care coordination outcomes.
  6. Describe the positive impact best practices across the continuum have on the overall population health.
