President - Jayme Ambrose, DNP RN CCM

Jayme Ambrose DNP RN CCM has been in the field of healthcare for over 20 years. She has earned a BSN and MSN from Arizona State University and her DNP from Case Western Reserve University. Her practice focus has been on on community health and care coordination. Jayme has extensive knowledge and experience in development and management of case management programs. She is currently the Chief Clinical Officer for the the Beech Group.

Vice President - Rachelle Haviland, RN

I have been an RN since 1994. My husband, Ramesh Ramaswamy, MD, & I started our internal medicine practice in 2000, & Doctor’s Choice Assisted Living in 2008. We bought our Assisted Living Locators franchise three and a half years ago and are proud to serve the East Valley and surrounding communities. I have also worked as a Case Manager, Infusionist, Research Coordinator, Camp Nurse, Wellness Director, Telemetry Traveling Nurse, and a variety of roles in Long-Term Care . My husband and I have a son who is going off to college next year.

Past President - Lesley Hunt, RN, MSN, CCM

Lesley Hunt is an RN case manager with Orchard Medical Consulting. She graduated from Arizona State University with a BSN in 1980 and University of Phoenix in 2003 with an MSN. Lesley has been a case manager for over 15 years in a variety of fields such as Medicare, commercial claims, and currently with Workman’s Compensation. She has been certified in case management for 13 years.

Lesley also works as adjunct faculty for University of Phoenix, Nursing program. She is also active in working with nurses in recovery from addictions by facilitating a Nurse Support Group and has done so for the past 20 years.

Treasurer - Rex Nelson, RN

James “Rex” Nelson is a Registered Nurse, practicing for over 26 years. A Case Manager with certifications from both the ANCC and the CCMC, Rex’s current role promotes safe and efficient patient transitions to post-acute care services for several hospitals and hospital systems across the United States. Rex’s nursing background began in critical care (ICU, ED, Flight Nursing) and over the past 15-years in Case Management, Workers Compensation, Utilization Review, and Revenue Cycle Management in the acute care and post-acute care settings. Rex has taught in many areas, including emergency services (EMTs, Paramedics, Trauma Nursing, and Physicians), case management and utilization review.

Rex completed Nursing School at El Centro College and holds a Bachelor’s of Applied Arts and Science degree in Emergency Administration and Disaster Planning from the University of North Texas, as well as a Master’s of Business Administration with an emphasis in Health Systems Management from Grand Canyon University.

Rex has been active with the Arizona Chapter of CMSA since 2005 and joined the board, as a Member-at-Large in 2009. Rex moved into the Co-Treasurer’s position in 2012, and has significantly updated the chapter’s financial recordkeeping.

Secretary - Wendy Dolfo, RN

Wendy Dolfo is a Registered Nurse with a 30-plus year career. She is currently employed as a Care Coordinator for Maricopa Integrated Health Systems in Phoenix. Her clinical experience spans a diverse spectrum of specialty areas, including outpatient oncology certified in intravenous infusion, high-risk nursery and pediatrics. Her Case Management expertise encompasses a more than 15 year time-span.

Wendy completed Nursing School at the University of Hawaii at Manoa in Honolulu, Hawaii. She is a native to the island of Oahu for most of her life, transplanted to Arizona in 2008.

Wendy is newly elected to the CMSA-AZ Chapter Board as Secretary this year. Wendy lives in Chandler, Arizona with her husband and children. Besides time spent with grandkids and many family pets, she and her family enjoy sharing the culture of Polynesia with the people of Arizona through their Polynesian entertainment business.
